(1) FT 8a - 5p Monday - Friday + on-call as assigned
Summary: To manage Skilled Nursing Facility residents and the Resident Assessment Instrument process in accordance with current Federal, State, and Local standards governing long-term care facilities, and as may be directed by the Long Term Care Director of Nursing to ensure that the highest degree quality care can be provided to the residents at all times.
Essential Job Functions:
Assess and review charts of NF/SNF residents.
Lead team to assure real time participation in interdisciplinary care plan process.
Participates in interdisciplinary care plan meetings with interdisciplinary team per care schedule providing input regarding resident care needs and goals.
Review NF/SNF residents with appropriate Medicare/Insurance reviewers for current and previous residents.
Complete Comprehensive Minimum Data Sets, 5-Day and Quarterly assessments for weekly scheduled residents.
Coordinate RAI process with interdisciplinary team.
Complete baseline care plan summary for new admissions.
Help IDT team update care plans quarterly/annually.
Review/Audit completion of MDS, CAA’s Care Plans, lock as required.
Transmit MDS assessments to CMS weekly.
Be familiar with State and Federal regulations in order to maintain survey compliance.
Be “on-call” on a rotation with nursing administration, as assigned
Assist with compliance rounds.
Complete all necessary/mandated paperwork for NF/SNF discharges (NOMNC’s, SNFABN’s).
Manage Medicare certifications for providers to ensure signatures are completed according to CMS guidelines.
Help Inter-disciplinary team in the submission of continued stay requests to Medicare Advantage Insurance companies for authorization of Skilled Services.
Be an advocate of residents and family’s rights.
Provides ongoing education to staff regarding MDS requirements and regulations.
Participates in quality improvement activities related to MDS assessments and resident care.
Participate in daily stand-up meetings.
Facilitates/manages restorative nursing program in Long Term Care Cottage environment.
All other duties as assigned.

Knowledge, Education, Skills/Abilities, Work Experience:
Knowledge: Knowledge and skills acquired through completion of State Certified, Nurse Program. Must be licensed as a Registered Nurse in the State of Montana. Must be knowledgeable of general, rehabilitative and restorative nursing medical practices and procedures and laws, regulations and guidelines governing long-term care. Must have certification or be willing to obtain certification for Resident Assessment Coordination within the first year of employment.
Skills/Abilities: Need to possess excellent communication skills both verbally and in writing. Must possess excellent nursing assessment skills and general organizational skills. Be able to interact effectively with residents, family members and co-workers. Must show ability to read and write in a legible and understandable manner. Must speak and understand English. Computer skills or a willingness to learn
Work Experience: Previous experience in hospital, long-term care facility, or other related health care facility.
Complexity/Difficulty:
This position requires the ability to function independently in performing assigned duties. Must be able to remain calm under stressful and urgent situations. The person needs to be able to respond to residents needs in a timely and professional manner, recognizing the patients’ rights. Incumbent performs job tasks following established procedures and reports problems or concerns to supervisor.
Working Conditions:
Normal health care environment.
Requires lifting up to 25 pounds on a regular basis and up to 50 pounds occasionally.
Bending, walking and standing for prolong periods of time.
